This policy applies to axiommedicalwellness.com.
1. About this policy
This policy explains how Axiom Medical Wellness, a medical wellness practice at 2272 95th St, Suite 125, Naperville, IL 60564, handles information collected through our website at axiommedicalwellness.com, our contact and consultation forms, and the phone calls, emails, and text messages that follow from them. In this policy, "we," "us," and "our" mean Axiom Medical Wellness, and "you" means the person visiting our site or contacting us.
By using our website or sending us your details, you agree to the practices described here. If you do not agree, please do not submit information through the site. You are always welcome to call us at (630) 287-8426 instead.
This policy does not cover the medical information created during your care once you become a patient. That is governed by federal health privacy law and by our Notice of Privacy Practices, which is a separate document you receive at the practice. See section 8. It also does not cover websites we link to, which run under their own privacy policies.
2. Information we collect
Information you give us directly
When you fill in a form, call, email, text, or book an appointment, we may collect:
- Your name, phone number, email address, and where relevant your mailing address.
- The service you are interested in, such as medical weight loss or IV therapy, and anything you write in a message field.
- Your consent choices, meaning whether you ticked the box agreeing to receive text messages or email from us, together with the date and time you did so. We keep this record because the mobile carriers require us to be able to show that you opted in.
- The date, time, and type of appointment you book.
- Anything else you choose to tell us, including health details you volunteer in a message or on a call.
Health information
Our services involve prescription medication, hormone therapy, and body treatments, so conversations with us often touch on your health. Before you are a patient, anything you volunteer through the website is treated as confidential and used only to answer you and arrange care. Once you are a patient, the clinical record we create is protected health information and is handled under the rules described in section 8.
Please do not send detailed medical history through the website form, by email, or by text. Ordinary email and SMS are not encrypted, and we cannot guarantee their privacy in transit. Share the minimum needed to get you booked, and save the detail for your consultation.
Information collected automatically
Like almost every website, ours is served by a hosting provider that keeps technical logs. These can include your IP address, browser type and version, device type, the pages you viewed, and the date and time you viewed them. We use this only to keep the site running, diagnose faults, and protect against abuse.
As of the date at the top of this page, our website runs no analytics software, no advertising pixels, and no tracking cookies of our own. If that changes, we will update this policy before the change goes live.
Information from other sources
If you message us through Instagram or Facebook, or leave us a review, we receive whatever that platform passes on, typically your profile name and the content of your message. If someone refers you to us, we may receive your name and phone number from them.
3. How we use your information
We use the information described above to:
- Answer your question and return your call, email, or message.
- Schedule, confirm, change, and remind you about appointments.
- Provide and follow up on the treatments you receive.
- Send you the text messages and emails you have agreed to receive, as described in section 4.
- Keep the records our profession and the law require us to keep.
- Take payment and handle billing.
- Improve our services, our website, and the information we publish.
- Keep our site and our practice secure, and prevent fraud or misuse.
- Meet our legal, regulatory, and professional obligations.
We do not use your information for any purpose that is incompatible with the reason you gave it to us, and we do not use it to make automated decisions about your care.
4. Text messaging and your phone number
We use text messaging to make care easier to arrange. This section explains exactly how our messaging program works and what happens to your number.
What we send
Axiom Medical Wellness sends appointment reminders and confirmations, replies to questions you have asked us, follow up about treatment you are receiving, requests for feedback on your visit, and, only where you have separately agreed to it, occasional news and offers about our services.
How we get your consent
We text you only after you opt in. You can do that by ticking the consent box on a form on this website, by giving written consent on a form at the practice, or by texting us first. Consent is never a condition of receiving treatment or of buying anything from us, and we will not text you simply because you called us once or because we hold your number for another reason.
Frequency, cost, and how to stop
- Our text messages come from (331) 295-3316. Saving it to your contacts makes them easier to recognize.
- Message frequency varies, depending on your appointments and the conversation you are having with us.
- Message and data rates may apply, depending on your plan with your mobile carrier.
- To stop messages, reply STOP to any text from us. You will get one confirmation that you have been unsubscribed, and then nothing further.
- To start receiving messages again after opting out, reply START.
- For help, reply HELP to any text, or call us at (331) 295-3316.
- Opting out of texts does not cancel your appointments and does not affect your care. We will simply reach you another way.
Your mobile information is never shared for marketing
This is the commitment patients ask us about most often, so we state it in full and without qualification:
No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
In plain terms: your phone number and the fact that you agreed to receive texts stay with us. The only outside parties that ever touch them are the technology vendors who deliver the messages on our behalf, listed in section 6, and they are contractually barred from using your information for their own purposes.
Delivery
Mobile carriers are not liable for delayed or undelivered messages. We cannot guarantee that every message reaches you, so please do not rely on text messaging for anything urgent. If you have a medical emergency, call 911.

6. Third party services we use
These are the outside services our website and practice rely on. Each one is bound by a contract that limits it to working on our behalf.
- HighLevel runs our contact forms, holds our contact records, and delivers our email and text messages. It may receive your name, email, phone, the message you write, and your consent choices.
- Acuity Scheduling takes online appointment bookings, and opens when you use a booking button. It may receive your name, email, phone, and the appointment you choose.
- Google Maps provides the embedded map on our home and contact pages. It may receive your IP address and your interactions with the map.
- Google Fonts delivers the typefaces this site is set in. It may receive your IP address, when the fonts load.
Our website is also served by a hosting provider, which keeps the technical logs described in section 2.
8. HIPAA and your health records
There is an important difference between this policy and the protection that applies to your medical record, and it is worth understanding.
- This privacy policy covers the information you give us through the website and through general inquiries, before and alongside your care.
- Our Notice of Privacy Practices covers protected health information, which is the medical record created when you become a patient. That notice explains how your health information may be used and disclosed, and sets out your rights to see it, get a copy, ask for a correction, and receive a list of certain disclosures.
You receive the Notice of Privacy Practices at the practice, and a copy is available on request at any time. Ask at the front desk, call (630) 287-8426, or email answers@axiommedicalwellness.com. Where the two documents ever appear to conflict on the handling of protected health information, the Notice of Privacy Practices and applicable health privacy law govern.
Where an outside vendor handles protected health information on our behalf, we put the appropriate agreements in place before it does so.
9. How we protect your information
We take reasonable steps to keep your information safe, including:
- Serving this entire website over an encrypted HTTPS connection.
- Limiting access to patient and contact records to the staff who need it to do their job.
- Choosing vendors that maintain recognized security practices, and binding them contractually.
- Keeping physical records secured at the practice.
- Reviewing who has access whenever someone joins or leaves the team.
No method of transmitting or storing information is completely secure, and we cannot promise absolute security. If a breach affecting your personal information occurs, we will notify you and the relevant authorities as Illinois and federal law require.
10. How long we keep information
We keep information only as long as we have a reason to, then dispose of it securely.
- Medical records are kept for the period Illinois law and our professional obligations require, which outlasts the end of your treatment.
- Enquiries that do not become appointments are kept for a limited period, in case you come back to us, and then removed.
- Records of your text and email consent are kept for as long as you are subscribed and for a period afterwards, because we may need to show that you opted in and when you opted out.
- Website logs are kept briefly by our hosting provider and then overwritten.
11. Your choices and rights
Choices you can exercise immediately
- To stop text messages, reply STOP to any message from us.
- To stop marketing email, use the unsubscribe link at the foot of any marketing email. We will still send email that is necessary for your care, such as an appointment confirmation.
- To stop marketing calls, tell us on the call, or ask the front desk.
Requests you can make to us
You may ask us to tell you what personal information we hold about you, to correct anything that is wrong, or to delete marketing information we hold. Contact us using the details in section 15 and we will respond promptly. We may need to verify who you are first. Requests about your medical record are handled under the Notice of Privacy Practices rather than here, because your rights there are broader.
Photographs
We photograph treatment results only with your separate written authorization, and we never publish an identifiable image of you without it. You may withdraw that authorization at any time, and we will stop using the image going forward.
Illinois residents
We do not collect biometric identifiers or biometric information as those terms are defined by the Illinois Biometric Information Privacy Act. We do not scan or store face geometry, fingerprints, or voiceprints. Illinois law also requires us to notify you if a breach compromises your personal information, and we will.
Residents of other states
If you live in a state with a comprehensive consumer privacy law, you may have additional rights over your personal information, including rights to access, correct, delete, and opt out of certain processing. We extend those rights to anyone who asks, wherever you live, and we do not sell personal information or use it for cross context behavioral advertising in the first place.
12. Children's privacy
Our services are intended for adults aged 18 and over. Our website is not directed to children, and we do not knowingly collect personal information from anyone under 18 through it. If you believe a child has given us information, contact us at answers@axiommedicalwellness.com and we will delete it.
13. Links to other websites
Our site links to places we do not control, including our booking system, our social media profiles, and occasional reference material in our blog posts. Following a link takes you somewhere governed by someone else's privacy policy, and we are not responsible for their practices. We suggest reading the policy of any site you share information with.
14. Changes to this policy
We may update this policy as our practice, our technology, or the law changes. When we do, we will change the "last updated" date at the top of the page. If a change materially affects how we handle your information, we will make that clear rather than relying on you to notice, and where the law requires it we will ask for your consent again.
